Scientists confirm new H7N9 bird flu has come from chickens

LONDON (Reuters) - Chinese scientists have confirmed for the first time that a new strain of bird flu that has killed 23 people in China has been transmitted to humans from chickens.
In a study published online in the Lancet medical journal, the scientists echoed previous statements from the World Health Organization (WHO) and Chinese officials that there is as yet no evidence of human-to-human transmission of this virus.

Kwok-Yung Yuen of the University of Hong Kong, who led the study, said its findings that chickens in poultry markets were a source of human infections meant that controlling the disease in these places and in these birds should be a priority.

"Aggressive intervention to block further animal-to-person transmission in live poultry markets, as has previously been done in Hong Kong, should be considered," he told the Lancet.

He added that temporary closure of live bird markets and comprehensive programs of surveillance, culling, biosecurity and segregation of different poultry species may also be needed "to halt evolution of the virus into a pandemic agent". :shock:

"The evidence ... suggests it is a pure poultry-to-human transmission and that controlling (infections in people) will therefore depend on controlling the epidemic in poultry," he said.

Yuen's findings do not mean all cases of human H7N9 infection come from chickens, or from poultry, but they do confirm chickens as one source.

The WHO has said 40 percent of people infected with H7N9 appear to have had no contact with poultry.

Other so called "reservoirs" of the flu virus may be circulating in other types of birds or mammals, and investigators in China are working hard to try find out.

China reports new bird flu case in Hunan province

(Reuters) - China on Saturday reported its first case of H7N9 bird flu in the southern province of Hunan, the latest sign the virus that has killed 23 people in the country is continuing to spread.

The official Xinhua news agency said the patient was a 64-year-old woman from Shaoyang city who developed a fever on April 14, four days after having contact with poultry. Her condition had improved with treatment, it added.

LONDON - The first scientific analysis of probable human-to-human transmission of a deadly new strain of bird flu that emerged in China this year gives the strongest evidence yet that the H7N9 virus can pass between people, scientists said on Wednesday.

Research published in the British Medical Journal (BMJ) analyzing a family cluster of cases of H7N9 infection in eastern China found it was very likely the virus "transmitted directly from the index patient (a 60-year-old man) to his daughter."

Experts commenting on the research said while it did not necessarily mean H7N9 is any closer to becoming the next flu pandemic, "it does provide a timely reminder of the need to remain extremely vigilant."

"The threat posed by H7N9 has by no means passed," James Rudge and Richard Coker of the London School of Hygiene and Tropical Medicine said in a commentary in the same journal.

The scientists who led the study stressed, however, that the virus has not yet gained the ability to transmit from person to person efficiently - meaning the risk is very low that it could cause a human pandemic in its current form.

The new bird flu virus, which was unknown in humans until February, has so far infected at least 133 people in China and Taiwan, killing 43 of them, according to the latest World Health Organization (WHO) data.

Most cases have been in people who had visited live poultry markets or had close contact with live poultry in seven to 10 days before falling ill.

More new cases are appearing, so I will give a brief update to catch us up from last summer, then focus on the quarantine in Hong Kong. All below are from ProMed-mail until noted different source.

China reports new human H7N9 bird flu case

A new human H7N9 bird flu case was reported on Wednesday [23 Oct 2013] in east China's Zhejiang Province, the 2nd in China this autumn [2013], according to the provincial health authority. l.

His is the 2nd H7N9 case in China this month [October 2013]. The 1st, reported on 15 Oct 2013, was also in Zhejiang. This patient, a 35 year old man from Shaoxing county, remains in a critical condition.

[If confirmed, this will be the 137th case of avian A/H7N9 virus infection in China

12/4 Hong Kong, China: A total of 17 people who had contact with an Indonesian domestic helper critically ill with the [avian influenza] H7N9 strain are being sent to a holiday village for quarantine -- the 1st time the procedure has been activated since the SARS outbreak in 2003. The 36-year-old helper was confirmed on Monday [2 Dec 2013] as the 1st human case of H7N9 [avian influenza] in the city.

7 doctors who had treated the woman at Tuen Mun and Queen Mary hospitals have upper respiratory symptoms but also tested negative. 2 private doctors in Tuen Mun who saw the woman still have to be tested. The Authorities said the risk of human-to-human transmission is low. :huh "There is so far no evidence of the virus spreading to other persons," said Centre for Health Protection controller Leung Ting-hung. The Hospital Authority, meanwhile, said it will not allow visits at isolation wards in public hospitals.

From a blog with which I am not familiar (Pissin' on the Roses), so caveat lector:

Update #1.

Seven health care workers are reported to have symptoms, this is in addition to the 4 house hold members who also have symptoms, these 11 people are reported to have tested negative in a rapid H7 test.
And, in an interesting testing distinction an additional 6 individuals are waiting for laboratory testing to return results. All 17 of these individuals are under quarantine and are undergoing antiviral treatment, an additional 200+ individuals are under observation and have been offered antiviral treatment.

It is important to note that patient zero tested negative 2 times before finally testing positive, and obvious H7N9 patients with full blown ARDS have also previously tested negative.


UPDATE #2 12/4

The situation is being called "EXPLOSIVE". Emergency rooms in Hong Kong are bursting with patients with Influenza Like Illness (ILI), there is a 15 hour wait to see patients. The Queen Elizabeth hospital was packed with more than 150 patients. Hospitals are starting to use temporary beds (meaning people are being admitted).

BREAKING UPDATE #3 12/4
Internal Hospital emails show that the #H7N9 infected Indonesian maid actually tested H7N9 positive on November 28th, FIVE FULL DAYS BEFORE THE PUBLIC WAS NOTIFIED OR QUARANTINE ACTION WAS TAKEN.

Hidden H7N9 Human to Human Spread In Hong Kong via Primary School Children

We strongly suspect (NOTE: this is an opinion, not a conclusion) Human to Human spread of H7N9 in Hong Kong, via Kowloon school children, tied to common bus routes. Currently the Creative Primary School is under intense scrutiny and testing from public health authorities as the 10 year old son of the H7N9 maid's employer attends there. We suspect the maid was actually infected by the children.

There has been a strange combination and concentration of Influenza Like Illness, Pneumonia, and Gastrointestinal symptoms in primary school children in the Kowloon section of Hong Kong; There was a resultant death on November 17th, with symptom onset around November 11th.. The pneumonia was identified as Streptococcus, and we suspect it was a coinfection with H7N9. Gastrointestinal symptoms are also strongly associated with H7N9.

Coinfection is suggested as Streptococcus Pneumonia rates and deaths have been unusually high in this group for the last few months, such an increase in amplitude would be expected with an H7N9 coinfection. Such coinfection would likely preclude H7N9 testing and make symptomatic diagnosis of H7N9 highly unlikely.

More follows, with a map of the elementary schools and bus routes, and a list of news sources from which this information was gathered.

That's Funny I've had gastrointestinal Problems as of December 2011 until Now and Flu Like symptoms ever since until Now! but I'm still alive the Intestinal Pain was so bad in my Lower Right addomen (just above and including my right hip) that I thought I had Appendicitis! I felt Horrible I was Hoping the world would end so I didn't have to live with it! I was Bedridden MOST of the time! I could barely Walk let alone breathe properly So much agony from my intestines thank F :censor nowadays it feels like a needle going into my hip and some mild burning and stinging. :( Not Fun!

Bird flu spikes ahead of big holiday in China

A spate of bird flu cases since the beginning of the year in China has experts watching closely as millions of people and poultry are on the move ahead of the Lunar New Year holiday, the world's largest annual human migration.

China has reported more than 50 H7N9 infections in 2014 after the strain jumped from birds to people for the first time last year. The virus remains hard to catch and most cases have been linked to contact with poultry, but scientists worry that could change if it mutates into a form that allows it to spread easily among people.

For those who track influenza, the holiday, which begins Jan. 31, is always worrying because it comes during the winter months when flu typically rages. Add that to hundreds of millions of people — and often birds — crammed together on buses and other forms of transportation going home, and it's always a bit of a gamble. China estimates 3.6 billion trips will be taken over the holiday season.

"This is the first winter we've seen H7N9. We are in uncharted territory," said Gregory Hartl, World Health Organization spokesman in Geneva. "We have seen an upstart in cases, which we are attributing basically to the fact that it's winter. That combined with a lot of movement of people in crowded trains with chickens could give rise to a lot more infections, but we've also seen in past years where it hasn't."

The first H7N9 cases were reported in late March near Shanghai, and more than 200 others have since been identified, including some 50 deaths. A 31-year-old doctor became one of the latest fatalities, raising fears he may have been infected at the hospital where he worked, but none of his patients or other close contacts have reported flu symptoms, according to the Shanghai Municipal Commission of Health and Family Planning.

There have been a handful of family clusters, but WHO says no sustained human-to-human transmission has occurred. However, scientists warn that the H7N9 virus has genetic changes that could help it infect mammals easier than other avian flus. Infected birds also do not show symptoms, making it harder to track the disease. Cases declined in China over the summer, which is typically slower for influenza viruses, after some local live poultry markets were temporarily closed.
